On or about November 16, 2025, 360 Dental PC identified unauthorized access to certain computer systems. An investigation determined that files containing patient information may have been accessed. The information may have included patient names and, in some cases, protected health information. There is no evidence of misuse at this time.
Written notice has been provided to affected individuals where possible. This notice is provided as substitute notice in accordance with HIPAA and applicable Pennsylvania law.
