Notice is hereby given that Petition for Termination of Parental Rights has been filed by Children and Youth Services of Delaware County asking the Court to put an end to all rights you may have to this child, Olivia R., bd. 2/1/21 (female). The mother's last name is a Slowik and the child was born in Delaware County.
The Court has set a hearing to consider ending your rights to this child. The hearing will be held before the Honorable Rachel E. Berry on April 3, 2025 at 9:00am. You have a right to appear at the hearing to contest the Petition. You are warned that even if you fail to appear at the scheduled hearing, the hearing will go on without you and your rights to this child may be ended by the court without you being present. You have a right to be represented at the hearing by a lawyer.
In addition, you are advised that you may have an option for an enforceable voluntary agreement under ACT 101 of 2010 for continuing contact following the adoption of your children between the adoptive parents and a birth parent if all parties agree and the agreement is approved by the Court.
You should take this paper to your lawyer at once regarding these Petition and to confirm the information necessary to participate in the hearing. If you do not have an attorney or cannot afford one, go to or telephone your local Bar Association/Legal Aide. The attorney that has been appointed to represent the mother is Jacquie Jones, Esquire at 610-874-1900.
